Abstract
One of the key challenges that anesthesiologists and intensivists face in their daily clinical practice is determining the optimal way to ventilate their patients, implementing strategies aimed at minimizing trauma and maximizing benefits. Ventilatory strategies for critically ill patients, particularly those suffering from ARDS, are frequently discussed at national and international meetings, especially when led by prominent experts in the field. For too long, mechanical ventilation during general anesthesia has been considered the “Cinderella” of the field. However, new ventilatory modalities available on modern anesthesia workstations, if properly applied, can positively impact ventilation and reduce postoperative pulmonary complications.
With this in mind, we are proud to invite you to this webinar featuring four leading experts in mechanical ventilation, providing participants with at least two major opportunities.
Firstly, over the past 10 months, three pivotal articles on ARDS have been published: the results of a consensus conference discussing the new “global” definition of ARDS (Matthay et al., AJRCCM 2024); the ESICM guidelines on Acute Respiratory Distress Syndrome, covering definition, phenotyping, and respiratory support strategies (Grasselli et al., ICM 2023); and the official ATS clinical update on practice guidelines for the management of adult patients with ARDS (Qadir et al., AJRCCM 2024). Despite the rigorous methodology based on data from relevant RCTs and the presence of common experts among the panels, the conclusions and guidelines have raised doubts and questions among readers. Prof. Luciano Gattinoni will critically discuss the new global definition of ARDS (2024), while Professors Alain Mercat and Davide Chiumello will provide commentary on the new definition and ARDS guidelines, highlighting controversial aspects and addressing ventilation management strategies (such as PEEP selection), with a strong focus on physiology.
Secondly, Prof. Carlos Ferrando Ortolá will emphasize the importance of a modern approach to mechanical ventilation during surgery, offering updates on lung-protective strategies and discussing an individualized intraoperative approach for optimal lung opening.
